Penetration where the tip of whatever is penetrating ends up out in the open, as with regular threading, but where the entry or exit points (possibly both) aren't generally counted as body orifices. These can include holes that are unusual but appropriate or artificial holes, either from some medical procedure (like tracheotomy), from dismemberment, from having detachable limbs of some kind.

While this differs from impalement in that the holes need to have already been there, this is much more likely to contain gore than the other types of threading by the nature of the tag.
