A ruff is a ruffled piece of fabric worn around the collar - those "frilly collar things" from the Renaissance. Sometimes a part of an ornate medieval or renaissance style dress worn by nobility or royalty.
Ruffs are also often worn by, and strongly associated with clowns, jesters, and the circus.
