You're really underestimating how much flesh there is between the perineum and the prostate. Also, there's load of loose skin in the area.

From someone who has done this exact piercing, and a few others to my own genitals, I can confirm it barely hurts at all. In fact, the only genital piercing I have felt any concerning amount of pain was a PA, and it only hurt for like 1 second. It is much more painful to pierce nipples or ears. I've pierced my scrotum a few times, and that didnt hurt 1 tiny bit. No soreness afterwards either. Frenum piercings don't hurt either. I usually find myself surprised the needle has even gone through. Fact is, that area of your body is incredibly pain tolerant, aside from the actual organ (testicles) of course.
