You can attempt to prevent any problems outright, but for folks who insist on stewing in it all day it's a matter of time.
Even if you aren't using them the plastic backing doesn't breathe, so over time you will end up sweating and that sweat can't evaporated through the plastic barrier.
A few ABDLs I've met have the same characteristic pimply upper thighs, because that's where moisture tends to build up and give you those skin conditions.
The preventative measure is to use baby powder (it's sort of a dessicant meaning it sucks water up into itself and also sort of a dry lubricant preventing the diaper from chafing too much).
Of course it's just dust, so you'd need to reapply often to continue getting the benefits otherwise it tends to rub off on everything, immediately.
The other option is to use lotion to create a wet barrier between your skin and any other WORSE liquids that could be clinging to your skin.
