The full picture makes more sense. The thumbnail version made it look like she had some weird thorn or spike nipples.

Well, when a clothespin is left alone, the clamp side is shut and the other side is open. But her nipples keep it from shutting, so the other side is closed. I guess it was a tight fit.

Artist seems to know how they work better than you do. If the jaws are open, the other end will be closed. And since the jaws are clamped down on nipples, and therefore mostly open, the other ends will be, logically, mostly closed. As we see here.
