Part of what makes passing Kidney stones so painful is that they're not perfectly round or smooth. In fact, they're very jagged and ridged which scrape painfully along your urethra tract as they're passing through. Plus dragons are magical in nature so I imagine some laws of physics and limitations are being ignored.
