My guess is that they were a "he" while alive but became a "she" after coming back in the new body.

tl;dr: she didn't know she was a "she," and doesn't even have the anatomical details (or even tone of voice) to prove one way or the other - but *somehow*, the kid knows. (oooo spooky mystery)

...also I should probably specify that this is (kind of) a guess; I've read the original story that the comic is adapting, but there are enough tweaks to the story that I'm not 100% certain of the details.
