For me, it's like an all encompassing surrounding bodily pressure thing. In theory, at least. Until it reaches the head, then I'm iffy. But it's really only good if it feels thick and like it would apply a decent amount of pressure. No dice if people make it look too watery. Again, in theory. Never actually done it myself.
