That's the doll that appears whenever a pokemon uses Substitute.

That's a pokedoll, an item used for escaping encounters, it was used to skip marrowak's ghost back in gen 1

Because the mad dummy from Undertale is at the very edge of the screen on the right side of the image.

There is also one ghost at the far left.
