It's underwear, it permits art to be enticing without it being entirely erotic in nature.
Then there's the folks that are into, for want of a better way to describe it, "primitives", or as it's often put "savages" or "tribals" for various reasons, some more, er, out there than others. The type meant to appeal to this audience is generally not actually a loincloth but more of a groin apron as it doesn't actually cover anything.
Then there's some of us that just are used to fundoshi and similar garments, and just don't view it as much different from swimwear or other "it's fuckin' hot out" or going swimming garments. It's clothing, the appeal and use cases match any other articles.

Sometimes the tease and anticipation is better than having full exposure right away. A loincloth leaves little to the imagination, exposing the thighs whether they're thick or muscular, framing them nicely, but covers up the area that people really desire. For some people this is turn on because, as mentioned, they enjoy being teased and potentially getting to see a character's bits. For me, I think seeing a dude's big ol' underwear bulge first can be hotter than him immediately whipping out the full hog and this lends to my appeal towards loincloths.

We all have things we don't like and the best
way I've found to deal with 'em are:
- Blacklist it
- Get an Element Blocker
- Cut it out of your day-to-day
If it's just a piece of fabric draped in front of someone's junk and doesn't offer any support whatsoever or hardly covers anything at all, it's like they're more naked than they actually are- the suggestion of clothing makes their nudity more apparent, like wearing nothing but shoes.
And there's the added risk of upskirts or being pulled aside, drawing a fine line between being clothed and nude at the same time that adds a certain thrill- it's doing the bare minimum to conceal what's behind it while also drawing attention to it, as Notkastar said.

Different people have different ways of finding certain things appealing about any little thing, including articles of clothing- that's more or less how fetishes work, after all
People in modern societies typically don't wear loincloths and the way they're often depicted isn't accurate to how they actually function in the real world, and I think that misinterpretation- culturally insensitive as it might be- has lent itself its own sort of exotic appeal to those who see it; leave it to them to better explain it than anyone who doesn't, haha

Actual loincloths are just underwear, they feel like you'd expect, depends on the material and how tightly you wrap them, but overall it's not too different from any other variety of smallclothes. They tend to be a bit warmer and thicker than elastic varieties.

Again, in the real world it's underwear. It's easy to make, given in it's simplest form a loincloth is pretty much the same in construction as a scarf. It's the same as any other underwear or swimwear. You'll primarily see it in Japan in this context, as much as a matter of tradition as simply being, well, "if it ain't broke don't fix it", though it's mostly older generations that are going to tend towards it.
Other modern usages are pretty much just ceremonial, not accounting for poverty or "fringe contact" humanity that exists in a primarily or entirely premodern lifestyle (Sentinel Islands, some parts of Africa and South America, etc.). It's also easy to make one in an emergency, should you somehow have no other options for clothing, as it uses relatively little fabric and covers the bare minimum required to not catch an indecent exposure charge in locales that do not permit naturalism.

Modern ones are, again underwear and are made accordingly. Silk and cotton are the most common. Barkcloth was the traditional material for primitive humans. The manufacture is quite literally as simple as making a long strip of your chosen textile. That's it. It's the same as a scarf. Have you considered just... using Wikipedia for this?
