Extreme thinness from absence of body fat and lack of muscle — a character that is essentially "skin and bones", skinny to an extreme and, oftentimes, unhealthy degree.
An emaciated character's ribs or entire rib cage will be visible under their skin, their pelvic bones may be visible as well, their abdomen concave, and their limbs thin and bony; the character's cheeks and eye sockets may even be concave.
"Emaciation" by definition is a sign of malnutrition and/or disease — however, some characters, especially ones intended to appear monstrous, are "naturally emaciated" in appearance without necessarily being starved or diseased.
This tag can also be used to describe apparently healthy, but extremely skinny characters — e.g. the third example image above. A "healthy" emaciated character will typically only have thin limbs, a narrow torso, and visible ribs.
