In the comics medic had to scoop out part of demo's brain to make him stop asking for it back he asks so often. he keeps asking for it back, medic regenerates his eyeball back into his face, and it keeps exploding out of his face every Halloween into Brain-in-a-jar eyeball monster, or flying bat eyeball moster, or, you know, MONOCULUS.

In fact, Medic mentions about five other Monoculus style eyeball monsters, and none of them are ACTUALLY MONOCULUS, So one may easily understand why Medic has given up putting his optical sphere back in his face socket, but he can easily have a new one if our resident satanic shaman ever had the wild hair to grow 'im a new one.
