There are a couple reasons that the tag history might show up like that:

• The first is purely a display issue, that is implications/aliases often falls onto the tag history of the next person that edits it even though the first person actually made the change (or the system did whichever way you want to look at it). This is mildly annoying when looking through the tag history to track changes but is otherwise harmless.
• The second is a glitch. Sometimes (for whatever reason) the implication/aliases don't get applied properly when adding a tag. This doesn't happen very often but when dealing with thousands of implications/aliases, hundreds of thousands of posts, and nearly 10 million tag edits, it can happen from time to time. Edits made during heavy load, multiple people tagging at the same time, duplicated requests, etc. may contribute to this, but I don't know for certain. Sometimes just re-approving an alias/implication will fix it, but that isn't done very often unless we suspect one might be causing problems.
