The right amount of stimulation in the right place can be nice. Too much and it'll quickly become the very opposite of nice. With some modifications and appropriate use, tools meant to administer stimulation for evil purposes can be used for good.

I've always wondered about that to be honest, m8.
What "Is" pain's connection to lust? Wouldn't getting hurt Hurt?

I think for a lot of peeps it's just fun to be in a sort of simulated peril scenario. you're never in any actual danger because you're with a person you trust to not actually hurt you.

pain usually sucks because it's associated with an injury, ailment, and the like, but pain can be fun when there's no actual (long-term) injury, just like fear can be fun when there's no actual danger (see: horror, rollercoasters).

If I'm right, I get the interest when it comes to close partners,

That makes me think there's more to it than roleplay. That there's a lust
in pain in the "right" way. Like a feel-good pain if that makes sense.

Most of those "Medieval torture devices" were art pieces invented during the Enlightenment to demonize the so-called "Dark Ages". Many would have been impossible to make with the skills and technologies available at the time like the Iron Maiden.

While true that a wooden rack or a whip isn’t beyond medieval craftsmanship, damn near all of the devices we picture today when we think "medieval torture equipment for sex dungeons" (Pear of Anguish included) are either 19th-century fabrications or massively exaggerated "reconstructions". The modern BDSM versions are riffs on those modern myths, not replicas of widely-used medieval tools. People sexualize symbols of powerlessness and punishment, so naturally the fantasy versions survive in kink while the historians keep facepalming.
