If someone were to want to take on the task of creating & populating a new tag, what are the rules & limitations of that? I've looked everywhere I can think & still come up blank. Any explanation would be appreciated.

So far as I've been able to tell, you find or post an image, adding on the tag, maybe make a wiki for it to explain what it is supposed to be. Make sure you change the category if it isn't a general tag. The community and staff decide whether or not to keep it, when they notice.
What tag are you thinking of?

There's no concrete guide to tag formation. The tags must follow the code of conduct and the tags must be in line with the category it is to be placed in.
If you're making a good faith effort to create the tags you won't get dinged for it
The help page gives good guidelines on stuff for each category
What SNPtheCat said, but also if it's a general tag try avoiding something that is too vague or too specific to be useful
For example
Someone got dinged for repeatedly tagging dozens of muscle groups that only a surgeon would know about, there were so many it caused visual pollution in the tag list and you couldn't find the actually useful tags in the post
