We have fth_transformation, and mth_transformation as tags, but would we just use the fti/mti_transformation tags for maleherm transformations, or would a tag like ftmh/mtmh suffice? I personally think its clumpy but. Idk

mtmh_transformation and ftmh_transformation would be the tags to use for male-to-maleherm and female-to-maleherm transformations. You shouldn't really ever tag intersex on its own, it should always use the type of intersex that's visible.
Aren't there a lot of old posts where it was used before we had all of the subtypes mapped out?

And futa is aliased to intersex since it can mean either herm or gynomorph, so someone tagging a post futa would result in the post just having intersex. But there should be no case where a character can be tagged intersex and not one of the subtypes, so there should always be a subtype tagged with intersex. If a post is missing a subtype, it should be added.
